A potential serious injury or fatality (pSIF) event is an incident or near miss with credible potential for serious injury or death that does not produce that severe outcome. Classification depends on the adopted framework, the event circumstances, and the safeguards relevant to the exposure.
For example, a forklift-pedestrian near miss may warrant pSIF review when the credible collision mechanism could cause crushing or other severe harm. Assessment should consider the energy involved, exposure, and effectiveness of controls. A pSIF is an event classification; a SIF precursor is a condition or signal that may indicate such exposure. A camera alert alone does not establish a pSIF without contextual review.
