Near Miss Reporting

← Back to Industry Terms

Near miss reporting is the recording and communication of an event that could have caused harm but did not produce that harm.

A useful report captures what happened, where and when it occurred, who or what was exposed, and the relevant conditions. The purpose is to identify contributing factors and improve controls. Internal reporting arrangements should encourage participation without blame and distinguish internal records from applicable external reporting duties.

For example, a worker reports a close encounter with a reversing forklift near a doorway. Available video and proximity records can support the investigation. AI Risk Radar can help teams review configured interaction events, but each detected event needs context before it is classified as a near miss.
