Lost time injury frequency rate (LTIFR) is a measure of qualifying lost-time injuries relative to hours worked, commonly expressed per one million hours. On that basis, LTIFR equals the number of qualifying lost-time injuries multiplied by 1,000,000 and divided by total hours worked for the same workforce and period.
For example, two qualifying injuries in 500,000 hours produce an LTIFR of 4.0 per million hours. The numerator follows the chosen reporting definition, which may include fatalities. LTIFR measures case frequency rather than days lost or potential severity; comparisons require the same case criteria and normalization basis.
